Submitting a Major Incident Ticket
Step 1.
Click Report an issue
Step 2.
Fill in the subject, be specific
Step 3.
Type as much information as you know in the description. The more information you provide, the better and we can respond. For example, include what you’ve done, what happened, and what’s currently happening.
If you have any screenshots, files, or pictures to attach, do so.
Step 4.
Check the box next to Production Critical.
At this point you are able to click submit. However, add if you can provide additional information, do so.
Step 5.
Specify the production critical category by choosing a line down or service down.
After completing all the required fields, which are denoted with *s, you can provide more information.
Step 6.
Select the date and then choose the time closest to when the event occurred
Select the Husco site it occurred at.
Type who the Service Desk can immediately contact, preferably someone who is on site.
If a computer is affected, what is its name? It should be labeled.
Step 7.
Click Submit.
